Use this service to cancel your provider registration.
Who can use this service
You can apply to cancel a provider registration using this service if you are either the:
- nominated individual at your provider
- responsible individual
- main partner
and:
- your provider is closing or has closed down its services.
When you can cancel your registration
You can only cancel your registration:
- from 30 days before the day you will stop providing your services
- any time after you have stopped providing your services.
Before you start
You will need to be able to tell us:
- your provider ID and name
- what measures you have taken to maintain the care of the people using your services
- who you have informed of your plans to stop your services
You need to have an up-to-date email address registered with us. You will need to access it to complete the application. You can update the email address you have registered with us online or by email.
You could be fined up to £2,500 if you give false or misleading information
