There are continued breaches of 4 regulations in relation to safe care and treatment, consent, good governance and staffing. We also identified 2 additional breaches in relation to fit and proper persons (recruitment) and safe environments. The registered manager did not always ensure they were working in accordance with the requirements of the Mental Capacity Act 2005. The service did not always ensure risks were mitigated as reasonably practicable. The service did not always ensure the premises were properly maintained and clean. The service did not always ensure staff received regular appraisals. Systems and processes had not highlighted all the shortfalls identified from this assessment.
