On the 18 and 19 of November 2024 we conducted an assurance visit at Winsford Grange in response to information of concern received shortly after the last comprehensive inspection which was conducted from 4 July to 19 July 2024 with the report published 11 October 2024. Enforcement action was taken following the July 2024 assessment. During the assurance visit we looked at 7 quality statements from the safe key question and 2 quality statements from the well-led key question in response to the information of concern received. We were not assured during the visits and reviewed further information off-site pertaining to effective management of safe care and treatment and governance. The provider did not demonstrate and evidence a learning culture following their service analysis of accident and incidents including wound care, to mitigate risks to people.

People were not always protected and kept safe through the observations made. The facilities and equipment were in the process of an upgrade and improvements were visible, however, equipment was not always clean, medical equipment was not maintained, assessed or audited. Managers did not make sure all staff received training in moving and handling of people and their competence was not always assessed. Staff did not manage medicines well and found continued concerns with medication management.

We found the provider continued to be in breach of regulation 12 safe care and treatment and regulation 17 good governance of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014, there is enforcement action currently active, therefore no further enforcement action will be taken.

People shared mixed feedback about the service and the new manager, several people shared the manager was approachable and friendly. A family member said, “The manager is OK – time will tell, and actions need to happen.” A family member shared they observed some improvements in the service, however, said, “It’s just what you are told, they are only surface changes.” A family member stated there were more familiar faces recently regarding the staff presence, they said this was an improvement. Another comment made by a family member was complimentary about their loved one’s care, “Her meds are great, she is very stable, I can’t fault them.” People and family members were complimentary about the food being served and people said they looked forward to their main lunches, a family member shared the food was nutritious and a menu to choose from. One person spoken with was not happy about the tone of one of the nurses when we spoke to them, this was shared with the home manager and was looked into, the same person said, “Wait till she gets the shower list out, I will tell her where to go, doing it in front of people, I need headache tablets at the end of the day.” Some people could not directly tell us about their experience, observations we made were shared at the time with the home manager.
