Our view of the service

Date of assessment: 26 and 27 February 2025. Manor House is a care home that provides personal care for a maximum of 16 people. There were 13 people living at the service at the time of our assessment. This assessment was prompted by concerns we received about the service. Our rating for this service has changed from good to inadequate and the service was in breach of 7 regulations relating to safe care and treatment, safeguarding, consent, safe environments, staffing, fit and proper persons and governance. Systems to monitor the quality of the service were not robust or effective. Risks to people were not managed well. People were at risk of harm relating to skincare, choking, malnutrition, falls and managing people’s behaviours. Care plans lacked clear guidance for staff. Medicines were not managed safely. Staff had failed to recognise and report safeguarding concerns to the local authority and CQC. Environmental risk had not always been identified and were not protected from infection control risks. People’s rights were not protected in line with the Mental Capacity Act, 2005. Staff were not recruited safely and there were not enough staff to care for people. Staff did not have the skills and knowledge they needed to keep people safe. In instances where CQC have decided to take civil or criminal enforcement action against a provider, we will publish this information on our website after any representations and/or appeals have been concluded. This service is being placed into special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we user our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide. We have asked the provider for an action plan in response to the concerns found at this assessment in relation to breaches of regulations 11,18 and 19.

People's experience of this service

Whilst some people told us they were happy with the care they received and felt safe, others described incidents and accidents where people were not receiving the care and support needed to keep them safe. Our assessment found care did not meet the expected standards which put people at risk of harm.

Most people told us there were not enough staff available to meet their needs or spend time with them. Comments included, “There’s not enough staff, they need more. They are always rushing with very little time to chat.” Our observations found there were periods during the day where people would be left with minimal supervision which led to incidents and accidents resulting in harm to people. People did not describe safe practice with regards to their medicines. For example, some people told us staff left their tablets with them and did not watch them take their tablets. People and their relatives did not express any concerns with the environment and told us the service was clean. However, one person told us staff did not always wear Personal Protective Equipment (PPE) when supporting them. This comment was reflective of what we observed during our assessment.
