This service was responsive. People were at the centre of their care and were involved in decisions made about them. Advocacy services were involved to support decision making and there was a range of health professionals involved to meet all health needs.

Equity in experiences and outcomes

During our visit we saw outcomes agreed with people which would enhance their day to day skills as well as setting out plans to do fun things in their lives. There was evidence of what was agreed and how people were involved in deciding what they wanted to do. We saw people choosing places to go and things that they wanted to learn. People’s health needs were met. People’s health was monitored and they were supported to access appropriate health services when needed. Family members told us about staff keep people involved in community activities. One family member told us about the support [person] receives to go out and how their loved one has been supported to go on holidays with the home staff team.

Staff told us about the wide range of health professionals they ensured people had access to. One staff member told us how they supported people to access their own GP, dentist, chiropody service and hospital services. The staff told us about the important contacts they work with in the local hospitals to ensure people have smooth pathways into hospital services when they are required. Staff told us how they support people to maintain healthy lifestyles. This included having menu meetings to talk about choice and also talk about what is and is not healthy. Staff told us how they support people to try new experiences in different types of food that reflect the diversity of society. They told us how they had themed nights exploring cultures from around the world and celebrate this with food, music and clothing from different cultural backgrounds.

The provider had processes in place to identify the outcomes people wanted to achieve. We saw how this was agreed and recorded from the outset and who was involved. The provider had a system in place to review each goal and to report back on how progress was being made. This method ensured people did not lose sight of what they wanted to achieve, and outcomes agreed could be modified if need be. We saw extensive person-centred information which provided real depth about each person and who they were and how to promote good outcomes. We saw a ‘listen to me’ document which stated who was important to each person and how to involve them in people’s care.
