During a routine inspection
The agency had systems for auditing the quality of services provided, however many of them were verbal, not documented, so there was a limited overall view of how the service was meeting people's needs. Some assessments and care plans were not clear, including assessments and care plans relating to skin care and risk of tissue damage. Issues identified in daily records were not always included in care plans. Some care workers reported different issues about how they were supporting people. Systems to ensure all staff were trained in safeguarding people from abuse, particularly financial abuse, were not fully developed. Checks on new members of staff needed development, such as probing gaps in a member of staff's previous employment.
