During an assessment under our new approach

Date of Assessment: 5 December 2024 to 13 December 2024. The service is a care at home service providing support to older people and people living with dementia. This was a responsive assessment to review the providers actions to our last assessment. We assessed six quality statement in safe, all the quality statements in effective, caring and responsive and two quality statements in well led. The service had 4 breaches of regulation from the April 2024 assessment in relation to safe care and treatment, staffing, fit and proper persons employed and good governance. In this assessment we found the provider had made some improvements. However, they remained in breach of safe care and treatment and good governance regulations. Specific risks to people were not always assessed, monitored and managed to keep them safe. Mental capacity assessments and best interest decisions were not always in place where people lacked capacity to make their own decisions. A new care manager had recently joined the service, they had created an action plan they were working on and had started to address the concerns identified during our assessment. Governance systems and audits were not always effective in identifying or addressing areas for improvement. However, we did receive positive feedback from staff and people using the service. Staff understood their roles and responsibilities and had the right skills, qualifications and experience to provide safe care and support. There was a good learning culture in which managers investigated incidents thoroughly. Staff treated people as individuals and made sure people understood their care and treatment to enable them to give informed consent. People were involved in decisions about their care and were treated with kindness and compassion. A person said, “I definitely feel safe, no concerns at all, I feel very satisfied with their care.” We have asked the provider for an action plan in response to the concerns found at this assessment.

During an assessment under our new approach

Date of assessment 22 April to 7 May 2024. We visited the office on 22 April 2024. We reviewed records offsite and spoke with employees and people and their relatives between 25 April 2024 to 3 May 2024. AMG Care Services Ltd is a Domiciliary care agency (DCA) providing personal care for people living in their own homes in the community. This was a responsive assessment due to concerns raised to us about the service. Not everyone who used the service received personal care. This is the first assessment of the service. The assessment did not cover all parts of our Single Assessment Framework. We reviewed quality statements in Safe and Well-led key questions only and have therefore not given the service an overall rating. We will carry out future assessments to cover other parts of the Framework and will update our website with our findings. We found 4 breaches of the legal regulations in relation to safe care and treatment, governance, staffing and fit and proper person employed. Risks to people were not always assessed, monitored and managed to keep them safe. Medicines were not always safely managed. The provider had failed to operate robust recruitment procedures, including undertaking any relevant checks. The provider had failed to ensure staff were trained to enable them to deliver safe and effective care and support to people. Governance systems and audits were not always effective in identifying or addressing areas for improvement. However, we did receive positive feedback from partners who work with the service regarding completing detailed initial assessments and being responsive and flexible to people’s needs. We have asked the provider for an action plan in response to the concerns found at the assessment.
