People’s needs and risks from harm were assessed together with input from their relatives and healthcare professionals. This was to minimise identified risks and help keep people safe. risk management strategies were developed in the care planning process to minimise these risks and to help keep people safe. Staff told us they found people’s care plans helpful in knowing how to support people appropriately especially when they were experiencing signs of anxiety or distress.

Assessing needs

People’s relatives told us their family members needs were assessed together with the person, themselves and health professionals to ensure the service was able to meet the person's needs and wishes. They said they were also involved with care planning and that people's care plans included their healthcare conditions as well as the care and support they needed.

People's individual risks were assessed and risk management strategies were developed to minimise these risks and to help keep people safe. Staff told us they found people’s care plans helpful in knowing how to support people appropriately especially when they were experiencing signs of anxiety or distress. Comments included, “Care plans help us to support people better”, and “Support plans are person centred and essential for staff to be able to assist people with their care.”

There were appropriate policies and procedures to do with risk assessments and care planning in place for staff to follow and which staff were aware of.

Relatives told us they were involved in people's care planning and care plan reviews. They said they were informed when people's needs changed.

Staff said they always asked people for their consent before offering support and carrying out care tasks.

There were appropriate policies and procedures in place regarding consent and care planning.
