Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had systems in place which supported a positive culture of safety. The providers systems in place examined incidents to determine any learning and this was shared with staff. The registered manager told us any learning from incidents would be shared with staff in their staff meetings and how they used peoples feedback to learn about how to improve their care. People felt able to raise any concerns with the provider and relatives confirmed they felt action would be taken if they raised any concerns. Staff told us how they would report any concerns about safety and described the actions they would take if there were any incidents or accidents to keep people safe.

Safe systems, pathways and transitions

The provider worked in partnership to provide peoples care. The provider had systems in place to support people whilst working in partnership with other agencies. Care plans reflected how other agencies were involved in peoples care. Relatives described how the provider worked with them as partners in people’s care and support. Staff confirmed they had all the information they needed about people in their care plans and records were kept about care delivery. Partners felt the provider had a good understanding of partnership working and shared information as needed in the wider care system.

Safeguarding

People were supported safely by staff who protected them from abuse. The provider had systems in place to ensure people were safe and concerns were shared with the appropriate body. Where incidents had occurred, the provider worked with local agencies to ensure people were safe. People told us they felt safe living at the service. One person told us, “I can talk to the staff if I am worried about anything.” Staff understood how to recognise abuse and had received training in how to safeguard people. They could describe the signs of abuse and how they would report any concerns about people’s safety.

Involving people to manage risks

People were supported to manage risks to their safety. The provider ensured peoples risks were assessed and worked with people to minimise the risks. The registered manager told us risks were assessed and reviewed when things changed and updated on a regular basis. Risk assessments were in place to manage risks related to mobility, skin care, personal care and nutrition and hydration. One relative told us how staff supported their relative to manage risks to their safety when they went out into the community to keep them safe. Staff provided care which ensured people’s needs were met safely and could describe the actions they took to maintain people’s safety. One staff member told us how they supported a person with their mobility.

Safe environments

Score: 3

People were supported to live in a safe environment. The provider had systems in place to identify and manage potential risks in the care environment. Equipment, facilities and technology supported staff with the delivery of safe care. Relatives told us people were supported to maintain their homes by staff. Staff told us care plans detailed the support people needed to maintain their home and highlighted any risks.

Safe and effective staffing

People were supported by enough experienced staff. The provider ensured staff had the right skills and experience. The registered manager told us they had a system in place to ensure staff training was kept up to date and there were systems to ensure staff were recruited safely. People told us the staff understood how to support them. One person said, “The support I get from staff is good, I am happy with it.” Relatives told us staff understood how to support people. One relative told us, “The staff are really good with [person’s name] they monitor and support them really well.” Staff told us they had an induction and received training to support them in their role and had regular opportunities to get support in their role through supervision and staff meetings, staff had opportunities to discuss any safeguarding issues which had occurred and received updates on people’s care plans in staff meetings.

Infection prevention and control

People were protected from the risk of infection. The provider had systems in place to minimise the risk of cross infection and took appropriate actions to control the risks. There were regular checks in place from the provider on staff practice to ensure they were following appropriate guidance. Staff could describe the actions they took to keep people safe such as handwashing and using protective equipment.

Medicines optimisation

People were supported to take their medicines as prescribed. The provider ensured staff were trained and policies and procedures were in place to safely administer medicines. Staff had received training in safe medicines practice, there were regular checks on the system to ensure staff had administered peoples’ medicines as prescribed. Relatives told us staff supported people to take their medicines as needed. Staff told us they had systems in place to ensure they understood what medicines people needed to have and how to administer them. Staff could describe how they supported people to have their medicines safely.
