Effective

Assessing needs
The provider did not ensure staff always had access to care records to ensure people’s needs were met according to their care plan. Alexandra House was in the process of moving care records to an electronic system. We found people who had recently moved into the service had their care records recorded on the electronic system. However, care staff did not have access or had not received training in the new electronic system. This meant care staff had no guidance in how to support some people at the service. This placed people at risk of receiving care and support not in line with their assessed needs. Where people’s needs had been assessed we found care records to be inconsistent, we found some care plans to clearly detail people’s needs, whereas others did not. For example, we found a diabetes care plan to be detailed and clearly demonstrated the persons needs. However, records we reviewed for another person living with a complex medical condition had almost none of their needs assessed. People who spoke with us, told us they felt permanent staff knew their needs well but temporary staff such as agency needed prompting in how to support them effectively.
