Effective – this means we looked for evidence people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. This is the first assessment for this newly registered service. This key question has been rated good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

As the provider had recently purchased the service, they were working through reviewing and updating care plans to ensure they were reflective of people's needs, at the time of inspecting the service not all care plans had been reviewed, however, the completed care plans under this provider were an accurate reflection of people's needs. Staff knew the people they supported well and understood their needs. We observed positive interactions between staff and people who lived in the service. Where people required their dietary intake monitoring, fluid and food charts were completed. However, some people and relatives told us they had not been involved in the devising of care plans and were not aware what these where.

Delivering evidence-based care and treatment

The service planned and delivered people’s care and treatment with them, including what was important and mattered to them. People’s nutritional and hydrations needs were met in line with their current guidelines. Staff were aware of people’s dietary needs. The management team and the kitchen staff were in the process of devising a new menu based on surveys completed by people who live at the service to ensure they are providing a menu to meet peoples preferences.

How staff, teams and services work together

The provider worked well across teams and services to support people. Information was shared with external professionals when required to ensure people did not have to tell their story twice. Referrals were made to the relevant professionals when required to ensure peoples needs could be met.

Supporting people to live healthier lives

The service did not always support people to manage their health and wellbeing, people could not always maximise their independence, choice and control. Staff told us they did not have the time to support people to get up when they wanted. One staff member said, “There is no showers for residents due to lack of staff.” One person told us, “Showers are few and far between there isn’t enough staff.” Some people told us they did not have anything to do. One person said, “I just stare at these 4 walls”.

Monitoring and improving outcomes

The service did not always routinely monitor people’s care and treatment to continuously improve it. They did not always ensure outcomes were positive and consistent, or they met both clinical expectations and the expectations of people themselves. The monitoring of care ensures people receive the care and support tailored to meet their needs. For some people their care plans had not been reviewed for a long period of time and were not reflective of their needs. This was discussed with the provider who implemented a review of all care plans through resident of the day, this included all aspects of their care including medication reviews.

The service told people about their rights around consent and respected these when delivering person-centred care and treatment. We observed staff members speaking to people prior to providing care and support to ensure their consent was obtained.
