During an assessment under our new approach

Date of Assessment: 19 November 2024 to 21 November 2024. Little Lever Health Centre and Little Lever Library is a GP practice and delivers services to approximately 3000 people under a contract held with NHS England. The National General Practice Profiles states that 88% of people are white, 8% Asian and 5% black, mixed or other. Information published by Office for Health Improvement and Disparities shows that deprivation within the practice population group is in the 5th decile (5 of 10). The lower the decile, the more deprived the practice population is relative to others. This assessment considered the demographics of the people using the service, the context the service was working within and how this impacted service delivery. Where relevant, further commentary is provided in the quality statements section of this report. The provider, Dr Thiruppathy Subramanian, has been registered with the Care Quality Commission (CQC) since April 2021. In June 2023 the practice moved to a new purpose-built health centre and library. The provider applied to register this location in August 2024, and the new location, which we visited as part of this assessment, was registered by CQC on 5 November 2024. Only the location changed; the provider, lead GP, and all staff, remained unchanged. We found 3 breaches of regulation relating to safe care and treatment, good governance and fit and proper persons employed.

This service is being placed in special measures. The purpose of special measures is to ensure that services providing inadequate care make significant improvements. Special measures provide a framework within which we use our enforcement powers in response to inadequate care and provide a timeframe within which providers must improve the quality of the care they provide.
