Date of assessment 14 January 2025 to 20 January 2025. Copper Beeches is a ‘care home’ providing personal care and support to older people and people living with dementia. At the time of the assessment, the service was supporting 13 people with their personal care needs. Copper Beeches was last rated requires improvement (published 20 June 2022). The report was published following CQC’s old inspection approach using Key Lines of Enquiry (KLOEs), prompts and ratings characteristics. This assessment has been completed following the Care Quality Commission (CQC) new approach to assessment; Single Assessment Framework (SAF). We have identified breaches in relation to safe care and treatment and governance. Safety risks to people were not always managed well and risks were not always assessed. Staff had not always followed their training when supporting people with falls which could have put people at risk of harm. The provider had not followed their own policy and processes after accidents and incidents of completing a reflective practice record. There were no processes or systems in place when incidents had occurred to review what had happened and prevent re-occurrence. Lessons were not learned from safety incidents. There was not clear and effective governance systems and processes.
