Assessing needs

Score: 3

We received mixed responses from relatives in relation to their relative’s admission and care planning process. For example, 1 relative told us, “I don’t know anything about [person’s] care plan. They drew it up. I was never involved in it and I don’t know whether or when it was reviewed.” Another relative told us, “[Person’s] care plan is probably on the [digital system]. I have not been involved in any review.” We raised this with the provider who explained how a resident of the day process is used where a person’s care plan is reviewed, and that as part of this process, families are also called to provide feedback. This was supported by feedback from another relative, who told us, “[Leaders] were welcoming. [Leader] was brilliant. When [person] was admitted, they spoke to me for over two weeks about the medication and went through everything on the phone. Every month they have ‘Resident of the Day’ and that person has a range of health checks.”

Leaders explained the process of assessing a person’s needs and suitability for the service. They told us, “The initial process can happen in lots of different ways, family members can come to look around, there have been some occasions where a person’s needs are more nursing, so we have signposted people to other services… We use a trusted assessor, who use our paperwork, and they would complete the assessment [of a person’s needs]. We have also invited people and their families for lunch, when you are assessing someone in own home you sometimes get a bit of a false image. We always like people to visit beforehand, we have had referrals come through but have sometimes declined these, it is important to push back if the setting isn’t right for a person.”

People had care needs assessments in place and this information was transferred to people’s care and support plans . There was evidence of care reviews taking place through the resident of the day process.
