Terms of this registration relating to carrying out this regulated activity
The Registered Provider must create a comprehensive service improvement plan to address the shortfalls in medicines management, safety of equipment, care records, quality assurance and internal governance. This plan must be sent to the Care Quality Commission by 10:00 am on 29 August 2024 and must include timescales for actions to be completed and details of who will be responsible for implementing these actions.
By 10:00 am on Thursday 29 August 2024 the Registered Provider must have reviewed all electronic and paper medicines administration records (MAR) charts and a conducted a full review of all service users’ medicines prescribed. You must identify any errors and report to safeguarding and or CQC accordingly. You must review the two systems (Electronic and paper MAR charts) you are currently using, to ensure your record keeping is contemporaneous, and prescribing directions are accurate. The registered provider must supply the Care Quality Commission with a copy of this review, by 10:00 am on Thursday 29 August 2024 detailing its findings and any actions taken to address identified shortfalls.
By 10am on Thursday 29 August 2024 the Registered Provider must review all medicines prescribed on a ‘when required’ (PRN) basis and ensure PRN protocols are in place for each of these medicines. The registered provider must supply the Care Quality Commission with a copy of these updated PRN protocols by 10:00 am on Thursday 29 August 2024.
The Registered Provider must ensure that all service user’s care records are reviewed immediately with a plan detailing the measures put in place to address any deficits and shortfalls identified, a copy of this review be sent to the Commission by 10:00 am on 29 August 2024. The review must: identify immediate risks associated with service user’s care needs and inform an urgent review and updating of care plans and risk assessments. These need to include risks in relation to swallowing, pressure sores, medical health conditions, moving and handling needs and equipment, falls, and any specific risks in relation to medicines, such as blood thinning medicines.
The Registered Provider must ensure effective quality assurance processes are established and implemented to improve the way you assess, monitor, and improve the quality of the service and protect service users from the risk of harm. Evidence of audits you have completed must be submitted to the Care Quality Commission by 10:00am on 29 August 2024. Thereafter, the registered provider must send a report to the Care Quality Commission on the first Monday of each month with details of completed quality monitoring audits and checks. This must include areas covered (including care records audits and medicines audits) your findings and any responsive actions, clearly identifying who is responsible and the timescales for this.
The registered provider must not accept any new packages or assist any existing service user with any new package of care requiring support with medicines administration without the express, written permission of the Care Quality Commission.
The registered provider must immediately implement an effective system for managing, administering and recording the administration of service users’ medicines. This must include ensuring that;
i) there is no inconsistent information;
(ii) steps are taken to identify and reduce the risk of error; and
(iii) staff have access to sufficient information regarding the service users’ needs, to ensure safe medication administration.
