At our last inspection under the old provider we rated this key question good. At this assessment we did not assess all quality statements within this key question. Therefore under our new methodology the overall rating for this key question remains good. While the overall rating for this key question remains good, the evidence we found for delivering evidence based care and treatment showed improvements were required with involving people in their care. Staff demonstrated good knowledge of people and their support needs. However, this was not always reflected in people's care plans. For example, care records included the monitoring of people’s weight, but improvements were required to ensure sufficient oversight and evaluation was completed to implement improvement measures where people’s weight had reduced. People received input from other health professionals. Reviews were seen within care files with social workers, mental health teams and GP’s.

Delivering evidence-based care and treatment

We received mixed feedback from people and relatives about their care plans and their involvement. Some people were not aware of their care plans whilst others understood they had a care plan in place. Comments included, “I don’t have a care plan, no, I don’t think so anyway." and, “I do have a care plan. I didn’t have much of a say in it though.” and “Yes I have a care plan and I contributed to it." One relative commented, “[Relative] has lost weight since she has been here. I have seen food down the front of her clothes and they don’t change it. She also can smell [of urine], which is unacceptable”.

Staff told us, they held regular resident’s meetings. However, we found processes failed to fully evaluate and respond, with outcomes and actions, to all the feedback received. Staff told us they had responded to feedback provided about menus and the re-decoration of rooms. Whilst menus had been updated and some room re-decoration had taken place there was no clear provider oversight to deliver outcomes in a managed way. One staff member said, "We have just started to do this, the bedrooms are currently being re-decorated, residents are currently being consulted about this." We observed the environment including people’s rooms required maintenance, cleaning and updating.

People’s weight was monitored monthly using a blanket approach to weighing people. 11 people had lost weight between January to June 2024. There was no recorded analysis seen for these weight losses. However, the manager was able to discuss the actions taken with the inspector. Food and fluid charts were consistently completed. Reviews were seen within care files with social workers, mental health teams and GP’s.
