The registered provider must ensure that by 9am on the 8 July 2024 that they significantly strengthen the clinical leadership, mentorship, and governance oversight arrangements at The Limes Medical Centre to support and drive improvement. The CQC requires an action plan by the 28 June 2024 outlining
the actions they are planning to take/have taken to improve their clinical leadership, mentorship, and governance arrangements.
The registered provider must confirm to the CQC by 9am on 8 July 2024 that a suitably qualified, competent, skilled, and experienced manager is available during practice opening times at The Limes Medical Practice, to ensure appropriate leadership, management and provide support to staff and service users. The registered provider must maintain oversight of the practice management and ensure there is sufficient managerial capacity. The CQC requires an action plan by the 28 June 2024 outlining the actions they are taking to ensure appropriate management is in place.
The registered provider must ensure that service users prescribed medicines that require monitoring and long-term condition medicine reviews have been carried out within timeframes dictated by National Institute for Health and Care Excellence (NICE) guidance at The Limes Medical Practice. All service users being prescribed medicines that require monitoring and patients with long term conditions must have had a review within the appropriate timeframes within four weeks of receipt of this notice.
a. The registered provider must provide an action plan to CQC on how they will develop and implement an effective system and/or process for the management of service users being prescribed medications that require monitoring by 12pm on 1 July 2024. The process should include but not be limited
to:
i. Identification, escalation and management i.e. reviews, follow-up and/or investigation takes place, in line with recognised guidelines.
ii. The registered provider must ensure that information about medications that require monitoring are correctly recorded in records to support service user safety. Any service users highlighted as requiring a follow-up must be actioned by 5 July 2024.
b. The registered provider must develop and implement an effective system and/or process for the review and management of service users with a potential diagnosis of diabetes by 12pm on 1 July 2024. The process should include but not be limited to:
i. Identification, escalation and management i.e. reviews, follow-up and/or investigation takes place, in line with recognised guidelines.
ii. To ensure that information about potential diabetes diagnosis is correctly recorded in records/care plans to support service user safety. Any service users highlighted as requiring a follow-up must be actioned by 5 July 2024.
