Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last inspection we rated this key question requires improvement. At this inspection the rating has changed to good. This meant people were safe and protected from avoidable harm.

At the last inspection the provider was in breach of the legal regulation in relation to governance. Improvements were found at this inspection and the provider was no longer in breach of this regulation.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Accidents and incidents were recorded and there was a positive culture of looking for lessons that could be learned or improvements that could be made. Staff were aware of procedures for reporting accidents or incidents. They told us lessons learned were shared with them to reduce future risk.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. Staff and managers demonstrated an understanding of the importance of sharing information and working closely with other organisations and health care professionals. The registered manager worked very closely with the local GP and had good communication links with them. They held weekly ‘virtual’ ward rounds as well as direct visits.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. Staff had received training in safeguarding people from abuse and understood their responsibilities. They were confident that any concerns they raised would be dealt with promptly and appropriately. People told us they felt safe living at the home. One person said, "I feel safe here. I think the staff are good. They seem trained enough and I trust them."

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Risks were well managed and risk assessments were person centred. A new electronic care records system was being introduced to improve the detail and accessibility of the care records. Staff understood how to support people in a way that ensured risks were identified and managed, whilst respecting people’s choices and individuality.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The building, equipment and technology were well-maintained. Staff received safety training. One said, “On my first day I had training before I did anything. I was impressed with that; maintenance know their stuff and take Health and Safety seriously.” A relative said, "It is clean. It is well maintained and decorated throughout."

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision, training and development. They worked together well to provide safe care that met people’s individual needs. Recruitment checks were in place prior to staff starting to work at the home. There were enough skilled and experienced staff deployed to provide people with the care and support they needed. Staff told us they felt very supported and that the training was good. They said, “The induction was thorough, the support we get is very good and I can’t fault the training” and “We have good support and supervision, we are well trained and can always talk to managers; they have an open-door policy.”

Infection prevention and control

The provider assessed and managed the risk of infection. Staff had received training in infection prevention and control. The home and equipment were clean and risks associated with infections were assessed and well managed. People told us the home was always clean. One person said, "It is a home from home. It is always kept clean. Staff do all that.”

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. Systems were in place to safely administer and store medicines. Stock levels checked were correct, including controlled drugs. Patch administration sites were rotated to ensure maximum efficacy and minimal skin irritation.

People’s records showed medicines were being administered safely, including time-sensitive medicines. People’s care plans were person centred and documentation was in place to ensure people were having their ‘as and when required’ (PRN) medicines if needed. Charts were in place to show people were having their creams applied correctly. Improvements had been made to the storage of creams. The deploying of secure storage cupboards in the remaining bedrooms was in the process of being completed.
