The registered provider must provide the Care Quality Commission with a copy of the signed and dated APO1 application that was submitted to Companies House to appoint a new Director within 5 working days of the application being submitted.
The registered provider must notify the Care Quality Commission of the Nominated individual, Registered Manager, Office Manager completing their notice period or having their employment terminated within 5 working days. The registered provider must inform the Care Quality Commission of any Director stepping down as their role as Director, within 10 working days of said change taking place, as well as provide reasoning for this change.
The registered provider must provide the Care Quality Commission with information to reflect what steps were taken before the appointment of a new Director to ensure that they are a fit and proper person, within 5 working days of the APO1 application being submitted to Companies House. The information provided should include but not be limited to:
a) A document outlining the organisational structure
b) Pre-employment checks to ensure that they are a fit and proper person
consisting of a CV, DBS check results and at least 2 references from
previous employers
The registered provider must notify the Care Quality Commission of any service users, that they intend to transfer or discharge from Oakfield Psychological Services Ltd, this includes sharing with CQC in writing how they have notified relevant stakeholders of the events. This must be at least 10 working days before a planned transfer or discharge is to take place, or within 24 hours of an urgent transfer or discharge.
The registered provider must ensure that at least one individual that holds the role of Registered Manager, Nominated Individual, Office Manager or Director be present at engagement meetings with the Care Quality Commission, that will take place on the first Friday of every month at a time that the registered provider will be informed of by the Care Quality Commission, a week before the proposed meeting; unless re-negotiated and agreed by both parties in advance of the meeting.
The registered provider must submit an action plan to the Care Quality Commission by 5pm on the first Monday of each month demonstrating how they are making the improvements detailed as necessary in the report of the assessment carried out in July/August 2024.
