Condition of this registration relating to carrying out this regulated activity

a) By 4 pm on 9th November, the registered provider must implement an effective governance system which ensures that:
i. There is oversight in the management of the maternity services;
ii. There are effective quality assurance systems in place to support safe and quality care;
iii. The risk and occurrence of incidents are properly identified and managed, to include an effective system of recording actions taken and ensuring learning from any incidents;
iv. Incident grading is reviewed to ensure it is accurate and in line with national guidance.
b) By 4pm on the 9th November, and thereafter on a monthly basis, the registered provider must report to the Commission providing details of the system put in place and processes they are to ensure it is effective, including any results of audits or other monitoring activity.

a) By 4pm on 9th November 2020, the registered provider must implement an effective system to ensure patients are escalated appropriately and in a timely manner between departments and clinical staff in order to support good multidisciplinary (MDT) working.
b) By 4pm on 9th November 2020, and thereafter on a monthly basis, the register provider must report to the Commission on system it has put in place and the steps taken to monitor the system in accordance with 2(a). The report must include the following:
i. Information that handovers and safety briefings are in place and that appropriate staff attend.
ii. Details of actions taken to ensure that briefings and handovers are effective.
iii. The results of any monitoring data and audits undertaken that provide assurance that the system is effective.
iv. Summary report to the commission of any incident with significant patient harm. The report should include any results of audits or other monitoring activity.

a) By 4pm on 9 November 2020, the registered provider must implement an effective system to ensure that staffing is actively reviewed and escalated appropriately to maintain safe staffing in the maternity unit in line with national guidance. b) By 4pm on 9th November 2020, and thereafter on a monthly basis, the register provider must report to the Commission on system it has put in place in accordance with 3(a). The report must include the following:
i. The results of any monitoring data and audits undertaken that provide assurance that the system is effective.
ii. Actions to mitigate the risk of reduced staffing if staffing falls below planned staffing and/ or national guidance.
