Patients were comfortable with staff and staff respected patients’ space. Where staff had spoken on behalf of patients, they checked with patients to ensure they had done this correctly. Leaders gathered and acted upon feedback from carers and had an up-to-date risk register in place. The environment was safe, clean, and allowed patients to access several communal areas. Staff had access to up-to-date ligature risk assessment and environmental audits in place that manager reviewed regularly. However, the service did not always manage or assess risks well. Staff did not effectively handover incidents and did not complete appropriate physical checks and neurological observations after patient incidents. Leaders were able to show they had identified their current handover process was not working and they were in the process of imminently updating this. They did not always act in a timely manner when patient risk or distress was identified. We found a breach in regulation under safe care and treatment. Staff were not effectively assessing risk after patient incidents. We have asked the trust for an action plan to improve carrying out observations post incidents.
