Equity in experiences and outcomes
People and relatives told us the provider focused on positive experiences and outcomes for individuals. Care packages were bespoke and focused on individualized plans of care.
Staff told us people received individualised and personalised care, designed around their needs, wishes and preferences.
Processes were in place to ensure equity in experiences and outcomes. Care plans were person-centered and tailored to people's individual needs and wishes. The provider assessed what format people preferred records in and adapted this according to their needs. Individual packages of care were coordinated and strong links with other professionals meant people were offered a range of therapies as part of their care and support.
