Arbory Residential Home is a care home providing accommodation and personal care for up to a maximum of 60 people. The home does not provide nursing care. At the time of our assessment there were 30 people using the service, most of whom were living with dementia. We carried out our on-site assessment on 16, 18, 19 and 23 April 2024. We assessed 18 quality statements. During our assessment, we identified 6 continued breaches and 1 new breach of the legal regulations. We found concerns around the management of medicines and infection prevention and control. The provider did not do all that was reasonably practicable to assess and mitigate risks to people. Systems in place did not ensure people received care that met their individual needs and promoted positive outcomes. We found concerns around the providers application of the Mental Capacity Act (MCA) and concerns with premises and equipment. There was not sufficient activity staff and staff were not always recruited safely. Governance systems were still not effective in driving improvement. This service is being placed in special measures as it has been rated Inadequate in the Well-led key question for the second consecutive inspection. The purpose of special measures is to ensure that services care make significant improvements within a specific timeframe. The provider is also required to provide us with monthly reports to support our monitoring of improvement. Following the assessment the new manager provided documentation of action being taken to strengthen quality and risk management in the service. The local authority also provided positive feedback about improvements they had found in the management of the service.

People's experience of the service

People living at Arbory Residential Home were in varying stages of dementia and many were not able to converse fully due to the progression of their illness, hence in addition to conversations we carried out observations and spoke to people’s relatives. Some relatives we spoke with told us they did not feel their relatives were always safe living in the home. Relatives did not always feel they were listened to when they spoke out. One relative told us they were moving their relative to a home that focused more on sensitive person-centred care. All relatives spoken with felt there were not enough activities to keep people engaged and entertained. Relatives told us they were involved in setting up people’s care plans but had not been involved since then to ensure they were kept up to date. Relatives mostly felt the environment was safe.
