Assessing needs

The service made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. People’s care plans were personalised and specific to each individual and risks were clearly identified. A member of staff told us, “We operate on individual centred care which enables the service user and family to express how best they would love us to care for them.”

Delivering evidence-based care and treatment

The service planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards. People’s care plans were regularly reviewed. Key information about people’s needs enabled staff to deliver person-centred care and support.

How staff, teams and services work together

The service worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services. There was good communication and partnership working between the service and local health and social care professionals that ensured people’s needs were met.

Supporting people to live healthier lives

The service supported people to manage their health and wellbeing to maximise their independence, choice and control. The service supported people to live healthier lives and where possible, reduce their future needs for care and support. People’s care plans clearly evidenced how people were supported to maintain their independence to support them to live healthier lives.

Monitoring and improving outcomes

The service routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves. People’s care needs were reviewed regularly to monitor and improve outcomes. The service sourced independent advocacy support for people when required.

The service told people about their rights around consent and respected these when delivering person-centred care and treatment. Care plans identified people’s personal choices and staff respected them.
