Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last inspection we rated this key question good. At this inspection the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this. People had their needs assessed and their choices listened to. People’s needs assessments formed the basis of their care plans and these were accurate and up to date. People’s care needs were regularly reviewed and any changes or updates that were required were made accordingly.

Assessing needs

People we spoke with told us they had been fully involved in having their needs assessed and compiling their care plans and risk assessments. One person told us, “Yes, [care manager] came round and we were involved in doing that.” People told us their care and support were regularly reviewed and their care plans were updated if changes were needed. One person said, “They call occasionally, and they have a folder here. They take it away sometimes and bring a new one to replace it.”

The management team told us that people and their relatives were fully involved in their needs assessments. They explained the assessments covered all aspects of health and social care that people may require, including mental and physical health and people’s methods of communication. Staff told us they understood people’s needs, because information from the initial assessments fed into their care plans, which were regularly reviewed and kept up to date. All the staff we spoke to said they knew people’s support needs and that everyone had clear and detailed care plans and risk assessments. One member of staff told us, “I feel confident because I have the support I need and I am very familiar with my clients.” Another staff member said, “Yes, I do read care plans and risk assessments especially when I go to a different client.”

The service had effective systems and processes for assessing people’s needs. Following referrals from the local authorities, appropriately experienced staff from the service completed needs assessments with people, to determine if and how they could be best met. We saw the needs assessments formed the basis of people’s care plans and these were accurate and up to date. We also saw that people’s care needs were regularly reviewed and any changes or updates that were required were made accordingly.
