The provider used incidents of concern to learn across their services. For example, following one incident, the providers policy had been reviewed and included the introduction of a mandatory risk assessment being completed for all people across the care homes. The provider’s quality team monitored risk assessments for people were not missed. As part of the learning process a training video had been introduced as part of the provider’s mandatory training to give staff clear guidance. The provider’s weekly bulletins kept staff informed about changes being implemented and gave staff guidance about dysphagia. A monthly governance meeting in all of the provider’s homes discussed incidents and learning. The outcome of these meetings was shared with the provider’s quality team who assessed the concerns and learning across all of their services. The registered manager and provider’s quality team were able to easily monitor people’s care from the electronic records and identify any areas that needed to be addressed. The registered manager had reviewed a recent incident and had supported their staff team. They had analysed the information and looked to see if there was any learning they needed to put in place.
