The service understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity. Relatives confirmed this, one relative told us, “They do involve other health professionals,” they then shared an example of when the staff team had supported a person to overcome a specific barrier to health. The manager told us, “We work with a multi-disciplinary team. Health care professionals, GP’s, epilepsy nurse, cardiology, and district nurses. It might be we might not touch base for a long time, but I will get in contact with them when needed. We want to make sure care plans are up to date with this information.”
