Terms of this registration relating to carrying out this regulated activity
The Registered Provider must only accommodate a maximum of 51 service users at Forest Manor Care Home.
The registered provider must not admit any new service users to Forest Manor Care Home without the prior written agreement of the Care Quality Commission. This includes any respite or emergency admissions of service users, any readmissions of previous service users, or where service users return to the location following admission to hospital.
The register provider must ensure within 7 days, an effective system for the assessment and management of environmental risks across the location is in place and used by managers of the service. Areas identified by the system must be completed within a reasonable timeframe.
The registered provider must ensure within 7 days, that effective infection control processes are in place. This must include but is not limited to staff training, ongoing staff competence to manage IPC risk, IPC of the environment and to reduce or prevent the risk of cross contamination of infections between service users using appropriate IPC methods and equipment.
The registered provider must appoint a clinically competent individual to complete an immediate review of risk assessments and care plans of all service users within the next 5 days and update each care plan accordingly. The review must include, but not be limited to what the risk is, how to mitigate such risk and be individualised and person centred. In particular, service users with risks pertaining to, but not limited to the following must be reviewed:
1. Personal care; including bathing, oral care, clothing and bedding needs.
2. Nutritional and hydration needs.
3. Skin integrity and repositioning.
4. Access to call alarms, PIR sensors and prevention of falls.
5. Manual handling needs.
The registered provider must immediately review their staffing levels within the home and develop a robust training plan for staff, to ensure there are sufficient numbers of trained and competent staff to meet the needs of service users at all times. This review must include but is not limited to how you have ensured there is enough staff deployed to meet the current needs of service users at Forest Manor Care Home taking into account the following: -
1. Service users who require support from more than one member of staff.
2. Service users who walk with purpose and because of cognitive impairment are at risk of falling, becoming disorientated or entering other
people’s private rooms or personal space.
3. Service users with manual handling needs.
4. Service users who require support for mental health conditions.
5. Service users who have skin integrity and repositioning needs.
6. Service Users who require support for nutrition and hydration.
The registered provider must immediately instruct a suitably qualified, skilled and competent registered nurse to undertake oversight of medicines management which will include a documented audit of storage, and administration of service user’s medicines and take action as required to improve medicines practice.
The registered provider must, within 7 days, ensure that all service users have received a full and comprehensive assessment of their personal care needs.
The registered provider must, within 7 days, conduct and document a formal audit of the quality and safety of care provision at Forest Manor. Your audit report should detail any remedial or immediate action taken along with any other action that you plan to take as a result of the findings. Where relevant it should include associated timescales and highlight who will be responsible to ensure that the required improvements are made. This includes and is not limited to:
1. CCTV, starting immediately
2. Care plans and risk assessments for all service users
3. Daily records for all service users
4. Environmental audits, starting immediately.
