Well-led
The service was not always well-led. The provider had failed in their legal responsibility to notify CQC of significant events that affect people who used the service. This was a breach of Regulation 18 of the Care Quality Commission (Registration) Regulations 2009. Quality assurance and monitoring processes were not always effective.

Governance, management and sustainability
Governance and management systems were not always operated in line with the provider’s legal responsibility. A person had fallen down a flight of stairs and hurt themselves. The provider had a legal responsibility to notify CQC about this incident but had failed to do s. They did notify us of this after the assessment once we had told them of their responsibility to do so. Quality assurance and monitoring processes were not always effective. The provider did not have effective systems in place for monitoring the quality and safety of the premises. Shortfalls were not always identified, and where they were identified, action was not always taken to address the shortfalls. For example the premises were known to be in a poor state of décor, but there was o plan in place to address this.
