Date of assessment: 14 August to 25 October 2024. The service generally demonstrated a good level of safety and well led practices. Incident reporting and investigations were thorough, contributing to a positive learning culture. While staff raised some concerns about operational challenges, the service actively addressed these through risk assessments, policy updates, and collaboration with other services. Safeguarding procedures involved clear escalation pathways, and the provider prioritised inclusivity through translation services and non-discrimination policies. Although some initial challenges arose from team mergers, staff felt valued and supported by management initiatives, including access to wellness resources. Governance processes were in place, although some equipment oversight issues required attention. Overall, the service demonstrated a commitment to patient safety, staff well-being, and continuous improvement.
