Equity in experiences and outcomes
Peoples care, and support needs were assessed before they moved into the service. Assessments covered all aspects of individuals care and support such as, mobility and moving and handling, nutrition and hydration, personal care, communication, mental health and medicines management amongst others.
The registered manager told us that health and social care professionals visited the service when required to help support people and staff to best manage their needs and risks. Records showed that when required referrals to health and social care professionals were made and the service worked in partnership with them to develop care and support plans reflective of people’s needs and risk. We also saw the service worked closely with the local authority to ensure people’s needs were met. The registered manager told us the community mental health team (CMHT) visited people when required to review their needs. As part of their review people’s care plans and risk assessments were appropriately updated.
Care plans were developed and documented within the providers electronic care planning system in line with people's assessed needs. Care plans documented people’s assessed needs and risks to ensure staff provided person centred care. For example, one care plan documented the person preferred to receive support from female care staff. Staff were knowledgeable about people's individual routines, personalities and likes and dislikes. They were aware of the importance of providing person centred care and offering people choice in relation to their care and support. People were consulted and supported to make choices and decisions for themselves. Staff promoted people's rights and worked within the principles of the MCA to ensure these were upheld.
