How staff, teams and services work together
People and their relatives confirmed when required information is shared with relevant healthcare professionals. A relative told us where two care agencies provide a service, information is shared effectively.
Staff told us they work alongside health professionals to support people effectively. Staff confirmed they work alongside external teams and services such as GPs, pharmacies and district nurses. They told us they report concerns and updated information to the office when needed.
Care records did not always evidence information and guidance provided by healthcare professionals. Some care records contained contradictory information regarding who was responsible for managing a persons need. Accountability and responsibility of the providers role was not clearly demonstrated in peoples care records.
