Responsive
The provider had systems and processes in place to understand the diverse health and care needs of people receiving a service. People had care plans in place. Some care plans were not as detailed as they could be to help staff understand people’s care and support needs.

Care provision, Integration and continuity
People received care and support from a mixture of staff, some people received live in carers who lived in their home and were present 22 hours per day. The service then provided other staff 2 hours a day to give the live in carers a break. Some people funded their own care and some people’s care was funded by the local authority. A person told us, “They are very helpful and will always accommodate any time changes that we may need.”
Staff told us that there was continuity of support for people receiving care and support from the service. Some staff had worked at the service for a long period of time. A staff member said, “I have regular people I visit as I have being going to 2 different service users twice a day.” Staff told us they were confident in recognising when people were not their usual self which may indicate if they were unwell. A staff member told us, “If the person has a medical problem and I think they need to see their GP, if they have full capacity I would talk with them and advise them to make contact with their GP and offer assistance to do so. If they did not have full capacity, I would inform the office who would take over this job.”
A professional involved with the service gave us positive feedback about the service. They said, “Caremark staff have shown a good understanding of the specific care and support needs of the resident. Feedback from residents indicates that care is being provided at the appropriate times and for the necessary duration to meet their needs.” Staff supported people well and took timely and appropriate action when people’s health changed.
The provider had systems and processes in place to understand the diverse health and care needs of people living at the service. The provider ensured care was joined-up, flexible and supported choice and continuity. Staff told us they supported people with activities and attending appointments in the community. People's care plans included details of partners and external services which people accessed or needed to access. Some care plans were not as detailed as they could be to help staff understand people’s care and support needs.
