Assessing needs
The judgement for Assessing needs is based on the latest evidence we assessed for the Effective key question.
Delivering evidence-based care and treatment
The provider had systems to ensure that people who used the service received person-centred care and treatment that was appropriate, met their needs and was reflective of their personal preferences.
The dentist confirmed, where applicable, they referred patients to a range of specialists in primary and secondary care for treatment the practice did not provide. However, the practice did not have a process in place for monitoring these referrals. Following the inspection, we saw that a log had been implemented, although improvements should be made to ensure that the log was suitable for ensuring all referrals were adequately monitored.
Staff had the skills, knowledge and experience to carry out their roles.
There was no system or process to manage, identify or dispose of out-of-date stock. We saw that multiple expired products were present in the surgeries. Following the inspection, we were told these had been replaced.
We looked at 5 patient care records. The information recorded in these patient care records was not always in line with recognised guidance. Staff told us they obtained patients’ consent to care and treatment in line with legislation and guidance, however this was not always recorded. Following the inspection, the practice told us that a template will now be used to ensure clinical records are in line with guidance.
We saw evidence the dentists justified, graded and reported on the radiographs they took.
The practice had systems for prescribing medicines, which was not always in line with current guidance. Antimicrobial prescribing audits were not carried out.
The practice had systems for referring patients with suspected cancer under the national Faster Diagnosis Standard.
