Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. At our last assessment we rated this key question requires improvement. At this assessment the rating has changed to good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. Staff told us, “We have a lessons learnt book in the staff room which we must read and sign. Learning is also brought up in staff meetings.” The manager completed analysis of incidents at the service, which they used to make improvements. People we spoke with said they felt safe living at Maun View, a person told us, staff supported them following incidents which made them feel safe.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. All people we spoke with told us staff visited them before they moved into Maun View to ensure staff knew their needs and wishes. A relative we spoke with said, “Staff spoke with me and my [relative’s] old carers to go through what they needed before they came to live here.” Staff told us, they completed a pre-admission assessment to ensure people’s needs could be met at Maun View. Staff gave us examples of working with other health and social care professionals to ensure all people were supported to move into Maun View safely, this included visiting people in hospital and in other care homes to complete an assessment of needs.

Involving people to manage risks

Staff worked with people to understand and manage any associated risks. Staff supported people safely which was in line with their assessed needs and wishes. People and those important to them were involved with care plan reviews. These reviews took place with a designated member of staff to ensure people remained safe. A person we spoke with said, “We do updates with [staff name] and we had a 2-hour session about the care plan, and we have a 6-month review in case anything needs changing.” We found staff had clear guidance in place to support people safely. For example, we reviewed records for a person who was at a high risk of falls, there was a detailed falls care plan in place. We found any risk reduction measures required such as a falls sensor were in place. This protected people from the risk of harm.

Safe environments

The provider ensured risks associated with the environment were managed. However, we found large gaps in some maintenance records relating to water safety. This meant the provider could not be assured these were completed. We fed this back to the management team who told us due to staff changes some records were missing. Staff discussed all water checks had been completed recently with no issues. The management team told us they would review their process to ensure all maintenance records would be checked to prevent re-occurrence. Safety checks such as annual checks of electrical equipment were completed by a trained professional. The home was safe in the event of a fire, all people had personal emergency evacuation plans in place in the event of an emergency. Staff completed regular fire drills to ensure they could safely support people to evacuate the building, should the situation arise.

Infection prevention and control

The service assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The home was clean and tidy. A relative we spoke with said, “My [relative’s] room is spotless the staff are quick to clean odours. Occasional odours are cleaned up as soon as staff see it.” People told us, they were happy with the cleanliness of the home and staff worked hard to ensure any issues were resolved quickly.

Medicines optimisation

Medicines were managed safely. People were supported by staff who followed systems and processes to administer, record and store medicines safely. Staff received training in medicines and had their competency assessed. People told us they received their medicines safely. For example, a person we spoke with said, “I get my medicines on time, staff tell me if there needs to be any changes.” A relative we spoke with said, “Sometimes if they are in pain, they ask for a strong painkiller and they get it quickly.” The management team completed medicines audits regularly to identify any errors or poor practice. This meant any errors or learning opportunities were identified in a timely manner.
