Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence. At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing their health, care, wellbeing and communication needs with them. We were told by both the registered manager and people’s relatives that reviews of people’s care happened when their needs changed. However, there was no formal process in place to review people’s care records other than when a change had been identified

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They used clinical tools provided by their electronic care recording system to monitor people’s care needs.

How staff, teams and services work together

Score: 3

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs with staff. Each person who received support from the service had a QR code displayed in their home with Blossoming Hearts Care Agency’s contact details. This could be used by emergency services to contact the service if needed.

Supporting people to live healthier lives

Score: 3

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support. The registered manager gave examples of when they had supported people with their nutritional and hydration needs to improve the person’s health outcomes. A person using the service told us that staff supported them to be as independent as possible by encouraging and enabling them to do the things they could do for themselves.

Monitoring and improving outcomes

The provider did not always routinely monitor people’s care and treatment to continuously improve it. They did not evidence that outcomes were positive and consistent, or that they met clinical expectations. However, the registered manager was able to tell us of people who had improved outcomes due to the input of the service.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. The registered manager said when people lacked capacity, they worked with families to gain their views and wishes. People told us that staff respected their views and wishes and always asked for consent before carrying out personal care.
