Effective
The review of patient records found that the records did not always facilitate continuity of care and allow another clinician to take over the care of the patient and some did not include sufficient information to understand the patient’s assessment and management of the patient’s condition, and this may have impacted on a patient’s health.

Assessing needs
The GP survey carried out from January to March 2024 found most patients found the doctors treat them with care and concern, and they had confidence in the health care professional, and their needs were met. However, the review of patient records found that the records did not consistently facilitate continuity of care and allow another clinician to take over the care of the patient and some did not include sufficient information to understand the patient’s assessment and management of the patient’s condition, and this may have impacted on a patient’s health.
The provider responded that improvements would be made in response to the findings of the assessment and has submitted an action plan.
As part of the assessment a number of set patient clinical record searches were undertaken by a GP National Professional Adviser. These searches were visible to the practice. These found patient records did not facilitate continuity of care and allow another clinician to take over the care of the patient. The patient records did not include sufficient information to understand the patient’s assessment and management of the patient’s condition.
