Governance, management and sustainability
The practice had governance systems, policies and procedures which were accessible to staff and reviewed on a regular basis. The manager had created a compliance system in the daily diary, which was accessible to all staff to ensure that tasks were assigned and completed in a timely way. During the assessment, staff were open to discussion and feedback. Systems and processes were embedded to identify and manage risks. Staff worked together in such a way that where the assessment highlighted any issues, action was taken to address these immediately. For example, replacing medical emergency equipment and obtaining advice from their radiation protection adviser. Staff were aware of the importance of protecting patients’ personal information. Staff password protected patients’ electronic care records, and paper records were stored securely and complied with General Data Protection Regulations. There were processes for investigating incidents and accidents, and for receiving and acting on safety alerts. The practice had systems and processes for learning, quality assurance and continuous improvement. This included undertaking audits according to recognised guidance. Audits did not include conclusions or action plans (where appropriate). We signposted the provider to nationally approved audit tools to support with this process. Concerns and complaints were responded to appropriately, and outcomes were discussed to share learning and improvement. Staff demonstrated an open culture in relation to people’s safety. Staff had clear responsibilities, and systems of accountability to support good governance. Staff feedback was obtained through meetings and informal discussions. They were encouraged to offer suggestions for improvements to the service. Feedback from patients, the public and external partners was collected to which the practice responded. The practice taken steps to improve environmental sustainability.
