Learning culture

People felt safe. One person told us that RBK Solution Ltd, “Provides a safe service. I am someone who always communicates if something is not working well and they correct anything immediately. Whenever I highlight something, they sort it out immediately.”

The registered manager promoted a learning culture within the service. This involved sharing with staff the lessons learned when things had gone wrong and areas for improvement identified through quality audits. This information was shared with staff via team meetings, supervision sessions, in phone calls and online.

The registered manager routinely audited the knowledge and skills of staff to ensure they were capable of safely meeting people’s needs. Where additional training was required, this was provided. Staff were encouraged to share what they had learned from training and care courses with their colleagues. This promoted a learning culture.

Safe systems, pathways and transitions

People felt safely and effectively supported during the transition process into the service.

The registered manager and office staff reviewed referrals and liaised with health and social care professionals, people and their relatives. This enabled the provider to ensure the safe transition of people into the service.

Where required, the provider made referrals to health and social care professionals and worked collaboratively with them through the assessment processes.

The registered manager assessed people’s needs before the provider provided a service. This meant the service had the capacity and skills to meet people’s needs safely and effectively.

Safeguarding

Relatives and people said the service was safe and well managed. One relative told us, “They provide safe service.”

Staff received training to safeguard people from unsafe and improper treatment. Staff we spoke with understood the signs that someone may be at risk of abuse and knew the prompt actions they needed to take to keep people safe.

The provider had an up-to-date safeguarding policy, and the registered manager and office staff were clear about their duty to immediately report any safeguarding concerns to the local authority. Staff were able to access this policy on a weekly basis at the providers office. People’s mental capacity was assessed as part of the initial and full assessments undertaken by the provider. How people made decisions was stated clearly in care records.

Involving people to manage risks

People and their relatives participated in risk assessments and made informed decisions as to how care and support should be safely provided.

The care staff, office staff and the registered manager understood people’s individual risks. Where changes to people’s risks were identified these were reviewed and reassessed and plans were put in place to mitigate them. This meant people were protected from avoidable harm.

At our last inspection we found that risks associated with people's home environment, moving and handling, skin integrity and nutrition had not been properly assessed. This was a breach of Regulation 12 (Safe care and treatment) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. At this inspection we found that sufficient improvements had been made and the service was no longer in breach of regulation 12. The registered manager undertook risk assessments with people and their relatives. These were reviewed regularly or when people’s needs changed. Care plans were in place detailing how people’s risks could be safely managed. The areas risk assessed included people's home environment, moving and handling, skin integrity and nutrition.

Safe environments

Staff worked with people and their relatives to ensure that their home environments were safe when providing care and support.

Staff undertook health and safety training and provided care and support to people in line with their training and people’s care plans.

The provider supported people with a range of risk assessments to ensure their environments were safe. These included assessments around moving and handling, eating and drinking and health and safety.

Safe and effective staffing

People and their relatives told us there were enough staff available to provide timely care and support. One relative told us, “They do have enough carers, and they always cover anyone on annual leave.” People and their relatives had confidence in the skills and knowledge of staff. One relative told us, “I have never doubted their skills so their trainings must be good.” Another relative said, “Staff are properly trained, and I have no concerns about them.”

The registered manager ensured that staff were safe and suitable to provide care and support. This involved a robust recruitment process which included checks against criminal records data bases and the lists of individuals barred from working with potentially vulnerable people. The provider also checked the employment history of potential staff and confirmed their identities and eligibility to work in the UK. New staff received induction, on-going training, and regular supervision and appraisal.

At our last inspection we found that staff did not have all the knowledge and skills they required to meet people's needs because they had not received all the relevant training. Additionally, we found that staff were not supported with any formal individual or group supervision meetings. This was a breach of regulation 18 (Staffing) of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014. At this inspection we found that sufficient improvement had been made and the provider was no longer in breach of regulation 18. The registered manager provided staff with supervision and appraisal and oversaw a training programme which ensured that staff skills and knowledge were up to date.

Infection prevention and control

People told us staff wore appropriate personal protective equipment (PPE) appropriately and followed safe hygiene practices. One relative told us, “Staff all use PPE. They always wash their hands before they do anything else, and I think their training has highlighted this to them.” Another relative said, “They wear gloves and aprons and always wash their hands.”

Staff told us they received regular infection prevention and control training and used this learning when supporting people. They said they always had access to sufficient supplies of personal protective equipment such as gloves, masks and aprons.

The provider’s infection prevention and control policy was up to date. Staff understood the provider’s infection prevention and control policy, which was discussed in meetings and available in the office, and followed good hygiene practices. The registered manager and office staff carried out observations of staff in people’s homes to confirm they provided support in line with their training.

Medicines optimisation

Score: 3

People consented to the administration of their medicines and thought medicines management was safe. One relative told us, “There’s a system in place and it is charted, and they do this correctly and always inform if in short supply.”

Staff told us that they received regular training in the administration of people’s medicines. The registered manager and office staff carried out regular observations to confirm that staff were administering and recording medicines in line with their training.

The provider had an up-to-date medicines administration policy, which the registered manager discussed with staff. Medicines records were audited to confirm they were completed correctly.
