Safe
The service had a proactive culture of safety based on openness and honesty, in which concerns about safety are listened to, safety events are investigated and reported thoroughly, and lessons are learned to continually identify and embed good practices. There was an open and honest learning culture where staff felt safe to raise issues and concerns. The design, maintenance and use of facilities, premises and equipment kept people safe. All areas were clutter free and facilities and equipment were maintained regularly to keep patients safe. Cohorting areas had been fire risk assessed and safe for use. The service made sure that the equipment, facilities and technology supported the delivery of safe care. However the service did not always detect and control potential risks in the care environment. Not all patients who required falls risk assessessments had them completed. The service did not have an audit system in place to monitor the completion of falls risk assesments until December 2023. Audit results since December were below the Trust's target of 90%. The service had enough qualified, skilled and experienced staff. They worked together effectively to provide safe care that meets people’s individual needs. However not all staff had received up to date falls prevention training.
