Effective
The assessment looked at one quality statement due to specific concerns raised. We found no issues within this area and scored it appropriately, however, due to the focused nature of the assessment we did not look at other quality statements and as such the positive outcome of the assessment did not change the overall rating

Assessing needs
We spoke with five staff members who undertook triage as part of their role. All staff had received appropriate training and were able to articulate fully the process for the recognition and escalation of the deteriorating patient. We reviewed ten patient records and found all had been assessed and escalated as appropriate in all records, we also noted no errors or omissions. Senior leaders were able to articulate how they monitored and maintained oversight of the deteriorating patient.
