Learning culture

Lessons were learnt to continually identify and embed good practice.

There were systems in place to ensure lessons were learnt. The leadership team told us about a previous incident that had occurred that resulted in a safeguarding investigation. They told us how they had changed practices due to this and they had implemented better methods of communication with people and their families. They explained how they would implement this practice to other incidents when needed.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored.

The leaders told us the processes that were followed when people started using the service. They explained that once a referral had been received, they would go out and meet with the person and their families and complete an assessment to ensure they could meet their needs before agreeing to the package. They told us they would then base people’s care plans and risk assessments on these assessed needs. We saw this process had been followed when people started using the service.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. The provider shared concerns quickly and appropriately.

All people felt safe being supported by staff. A relative told us, “It gives me peace of mind, knowing that carers visit my mother.”

There were systems in place to ensure safeguarding concerns were identified, reviewed and investigated. Staff were able to tell us the action they would take if they were concerned about a person and confirmed they had received training.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them.

People were happy with how their risks were managed. One relative told us, “My relation is at risk from falling. The carers make sure everything is within her reach; they are aware that she is at risk from falling”.

There was a system in place to ensure people had care plans and risk assessments that were reflective of their current needs. These plans considered people’s mobility needs, health conditions and any known risks such as falls. These were reviewed regularly or when changes had occurred.

Safe environments

The provider detected and controlled potential risks in the care environment.

There were processes in place to ensure environmental risks in people’s homes and surrounding areas had been considered. We saw all people had individual risk assessments in place that related to their environment.

Safe and effective staffing

The provider had systems in place to ensure there were enough staff available to support people.

All people and relatives we spoke with felt there were enough staff, and that staff had the skills and training to look after them well. One person said, “I am not able to walk, they transfer me safely from bed to chair, they use a slide board if required, they know what they are doing.”

There were systems in place to ensure there were enough staff available to support people at the right times. Staff were allocated their rotas in advance so they knew who they would be supporting.

Staff had received the relevant pre-employment checks before they could start working with people to ensure they were safe to do so.

Staff had received training to ensure they had the relevant knowledge and skills to support people. This included training that was relevant to people’s individual needs.

Infection prevention and control

The provider assessed and managed the risk of infection.

People and relatives raised no concerns with infection control and all people told us staff wore Personal Protective Equipment (PPE). Staff confirmed they had enough PPE available, and they had received training in this area. There were processes in place to ensure staff protected people from the risk of cross infection.

Medicines optimisation

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences.

People and relatives raised no concerns with how their medicines were managed. A relative said, “Staff hand my relation her medications in a pot and ensure that she takes them. Her medication is stored in a safe place because she is confused and would be at risk of taking her medication unsupervised”.

People received their medicines when needed. Staff had received training, and their competency was checked to ensure they were safe to administer medicines to people. Monthly checks were completed on medicines administration records to ensure any concerns were identified and actioned.
