At our last assessment we rated this key question good. At this assessment the rating has remained the same. This meant people’s needs were met through good organisation and delivery.

Care provision, Integration and continuity
The service understood the diverse health and care needs of people and their local communities, so care was joined-up, flexible and supported choice and continuity.
Partners told us staff in the service worked well and jointly with them to ensure good outcomes for people. People were supported to live healthier lives through regular access to health care professionals such as their GP, dentist or optician. We saw guidance from healthcare professionals was incorporated into people's care plans and was followed.
Some people required support to make decisions due to their support needs. The service arranged for an advocate to ensure people’s rights were protected and respected.
