People received personalised care and support that was tailored to their individual needs. Care plans were regularly reviewed to identify if the care and support being delivered continued to meet people’s needs. People were encouraged and supported to pursue their interests and maintain links with the community. Information on how to communicate with people was included in their care plans. People and staff were treated well and respected regardless of their abilities, background, lifestyle, values, beliefs and their cultures were respected.

Person-centred Care

Care plans showed people likes and dislikes, when they wanted to get up in the morning and when they wanted to go to bed, this also included place they liked to go to and people who were important to them. People’s care plans had appropriate information for staff to follow and helped give them guidance on how to support people safely.

Staff told us that they were also involved in supporting people to keep their care plans updated. For example, a staff said that they met with people in key worker meetings to help them with achieving new goals.

We saw positive interactions between people using the service and the staff supporting them. Staff were clear on how people like to be supported.

Care provision, Integration and continuity

We saw during our visit that staff provided positive care to people and kept them safe.

The management team told us how they worked with people to move into the home and ensured their choices of how they wanted their room decorated and what furniture they wanted. People were also supported to bring their own furniture with them if they chose too.

We received positive feedback from partners about the service.

During our visit we saw evidence that the home worked with GP, and actions were taken by staff on advice received.

Providing Information

We saw that the home had information in written and pictorial format which helped people understand different information.

Staff told us that they supported people to read their post and complete information on forms as needed.

We saw evidence that residents and families’ meetings, were held. The minutes of those meetings were made available to those unable to attend.

Listening to and involving people

We saw that the provider sent surveys to relatives asking them for feedback on how the service was doing.

Staff told us they were happy with the way that the service was managed. A staff member said they felt supported by the management team.

We saw that that the provider had a number of ways of seeking feedback from people, relatives, staff and stakeholders.

Equity in access

We saw that the home was purpose built and fully accessible. The service had an action plan in place to ensure that the building was well maintained.

Staff confirmed that they had received equality training, and that the provider was supportive in this area for people and staff. The director also told us that their aim was to treat people as individuals.

We received positive feedback from relatives and from health professional which highlighted that the service was supportive and had no concerns.

We saw evidence that the provider supported people to have the access to care they required that meet their individual needs.

Equity in experiences and outcomes

We saw that the service catered for people's dietary needs. This was recorded in people's care plans and on the menus.

Staff told us that they supported people to have different choices that meet their individual culture needs and personal choices.

We saw joined-up working with the provider and with other partner agencies such as GPs and hospitals.

Planning for the future

We saw evidence people's care plans had information in place regarding how they would like to be supported if they were to receive end of life care.

During our visit the provider told us currently no one was receiving end of life care.

We saw that care plans were being reviewed on a regular basis and were updated when needed.
