Safe – this means we looked for evidence that people were protected from abuse and avoidable harm. This is the first assessment for this newly registered service. This key question has been rated good. This meant people were safe and protected from avoidable harm.

Learning culture

Score: 3

The provider had a proactive and positive culture of safety, based on openness and honesty. Staff listened to concerns about safety and investigated and reported safety events. Lessons were learnt to continually identify and embed good practice. The provider had systems to investigate and analyse incidents, accidents and complaints. Information gathered during investigations had been reviewed to look for potential trends and patterns. Wider learning was shared with the staff team.

Safe systems, pathways and transitions

The provider worked with people and healthcare partners to establish and maintain safe systems of care, in which safety was managed or monitored. They made sure there was continuity of care, including when people moved between different services. When people started receiving a service, staff supported them to be involved in deciding what care they needed. This helped ensure people received the care they wanted.

Safeguarding

The provider worked with people and healthcare partners to understand what being safe meant to them and the best way to achieve that. Staff concentrated on improving people’s lives while protecting their right to live in safety, free from bullying, harassment, abuse, discrimination, avoidable harm and neglect. The provider shared concerns quickly and appropriately. People told us were safe. A person said, “I feel safe, I have no concerns.” Safeguarding concerns were referred to the local authority safeguarding team and investigated. Staff had completed safeguarding training and knew how to raise concerns.

Involving people to manage risks

The provider worked with people to understand and manage risks by thinking holistically. Staff provided care to meet people’s needs that was safe, supportive and enabled people to do the things that mattered to them. Care plans included information about people’s individual circumstances to guide staff about how to keep people safe. Where specific risks to people’s safety had been identified, risk assessments were carried out to help minimise the risk of potential harm.

Safe environments

The provider detected and controlled potential risks in the care environment. They made sure equipment, facilities and technology supported the delivery of safe care. The provider completed a range of environment risk assessments to help promote a safe environment for people and staff. Staff felt the service was a safe place. A staff member said, “They (people) are definitely safe. We have hourly safety checks every night.”

Safe and effective staffing

The provider made sure there were enough qualified, skilled and experienced staff, who received effective support, supervision and development. They worked together well to provide safe care that met people’s individual needs. People gave positive feedback about their care and felt staff had the appropriate skills. The registered manager told us they monitored staffing levels and adapted them according to people’s needs and planned activities.

Infection prevention and control

The provider assessed and managed the risk of infection. They detected and controlled the risk of it spreading and shared concerns with appropriate agencies promptly. The provider had specific policies and procedures to promote good infection prevention and control (IPC). Staff had completed training in IPC.

Medicines optimisation

Score: 3

The provider made sure that medicines and treatments were safe and met people’s needs, capacities and preferences. The provider had policies and procedures to ensure medicines were managed safely. However, the 2 people receiving a regulated activity did not receive support from staff with their medicines.
