Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

At our last assessment we rated this key question good. At this assessment the rating has remained good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them. There was a person-centred approach to care planning and support. People had person-centred, positive behaviour support (PBS) plans and the staff we spoke with knew people well and were able to describe their needs and how best to support them. Staff were aware of the importance of involving people in all aspects of their care and support and making sure they were meeting people’s assessed needs. Care plans included information about people's preferences and were in an easy to read format. This made it easy for people to understand them. Staff told us people were supported to be involved in their care plans and made contributions to them. Health action plans were in place for people. They detailed their health needs and allergies. Hospital passports were in place and informative. They included all the information hospitals would need to know about people including things that are important to people and their likes and dislikes.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment. The registered provider, the registered manager and staff were passionate about supporting people in the least restrictive way and always considered and acted in people's best interests.

During our visits we saw staff seeking consent and people making choices about their daily activities. A range of communication tools were in place to support people to make and communicate their decisions. Staff demonstrated a good understanding of the principles of the Mental Capacity Act (MCA) 2005 and people were supported wherever possible to make their own decisions. Where people were unable to make their own decisions, records of best interest meetings and specific decisions were in place. People were enabled to access support from independent advocates. The provider had systems to make sure any restrictions in place were lawful and monitored.
