Effective – this means we looked for evidence that people’s care, treatment and support achieved good outcomes and promoted a good quality of life, based on best available evidence.

This is the first assessment for this newly registered service. This key question has been rated good. This meant people’s outcomes were consistently good, and people’s feedback confirmed this.

Assessing needs

The provider made sure people’s care and treatment was effective by assessing and reviewing their health, care, wellbeing and communication needs with them.

Assessments were completed prior to support starting and information collected transferred through to care plans. Care records were person centred and detailed how a person wanted to be supported as well as information around capacity and communication. Regular reviews of care plans were seen to take place.

Delivering evidence-based care and treatment

The provider planned and delivered people’s care and treatment with them, including what was important and mattered to them. They did this in line with legislation and current evidence-based good practice and standards.

Care records showed people were given choice and control about their care and support. The service responded to people’s care needs appropriately.

How staff, teams and services work together

The provider worked well across teams and services to support people. They made sure people only needed to tell their story once by sharing their assessment of needs when people moved between different services.

We saw evidence in care records of involvement from other health care professionals and appropriate follow up by staff when people needed health interventions.

Supporting people to live healthier lives

The provider supported people to manage their health and wellbeing to maximise their independence, choice and control. Staff supported people to live healthier lives and where possible, reduce their future needs for care and support.

Evidence of joint working with partners was available.

People received support from visiting healthcare teams. We saw evidence in care records and other documentation that support recommendations were communicated and implemented.

Monitoring and improving outcomes

The provider routinely monitored people’s care and treatment to continuously improve it. They ensured that outcomes were positive and consistent, and that they met both clinical expectations and the expectations of people themselves.

The provider had systems in place to gather feedback from people and relatives on their care and had systems in place to monitor support provided including on site observations.

The provider told people about their rights around consent and respected these when delivering person-centred care and treatment.

Staff received training in the Mental Capacity Act and understood the importance of ensuring that people fully understood what they were consenting to and the importance of obtaining consent before care was delivered. One staff commented, “Always ask consent from service users. Where service users cannot speak, they can use hand gestures, or they can nod or use facial expressions.”
