The registered manager told us they knew people’s needs well and had taken time to get to know them and carried out a full assessment of needs prior to starting the care and support package.

The lack of detail in risk and care plans meant staff did not have enough guidance to provide care in line with people’s assessed needs. The provider did not have a robust system in place to ensure information was recorded in enough detail for staff to follow. The provider did not provide staff with consistent training to be effective in their role. Overall, issues identified at this assessment could have been picked up by the provider if there had been a more robust monitoring and auditing system in place. The above outlined issues could place people at risk of harm.
