Equity in experiences and outcomes
Staff and leaders actively listened to information about people who are most likely to experience inequality in experience or outcomes and tailored their care, support and treatment in response to this. One person’s health deteriorated quickly and a relative told us how proactive staff had been in identifying and acting on this because they knew the person’s health needs well and what action they needed to take. Another person was being supported by staff in a flexible way to ensure they continued to be involved in social activities that were important for them and enhanced their wellbeing. The registered manager told how the service continued to support people for a period of time when they were admitted to hospital; this was done in partnership with hospital staff and to ensure the best possible outcomes for people.
