People’s needs were assessed before they moved into the home. Care plans and risk assessments were regularly reviewed to ensure they continued to reflect people’s needs.

Assessing needs

Relatives told us they and their family members were involved in their family members’ assessments and the development of their care plans. Relatives said care plans were reviewed regularly to ensure they continued to reflect their family members’ needs. One relative told us, “I attended the meetings where the care plan was established. And it does get reviewed; we go through all the necessary things.”

Staff said they had access to care plans which contained detailed information about people’s assessed needs and their preferences about their care. Staff told us it was important to understand people’s individual preferences to ensure their care was provided in the way they wished. One member of staff said, “The care plans are very good and detailed with people’s preferences. We really try to get to know everything about everyone here and make sure everyone is comfortable with the support and how we do it. We try to understand best what people enjoy.”

People's needs were assessed before they moved into the home and kept under review. Assessments addressed areas including health conditions, mobility, eating and drinking, medicines, and personal care needs and preferences. Assessments also recorded any risks people faced and how these should be mitigated.
